This Privacy Policy explains how Lifeline Health (“Lifeline Health,” “we,” “us,” and “our”), including Lifeline Health Florida, collects, uses, and protects information through our websites, including lifelinehealth.org and lifelinehealthfl.org, and related online forms and services (the “Websites”).

This Privacy Policy covers information collected through the Websites. The privacy of your protected health information (PHI) as a patient is governed separately by our Notice of Privacy Practices, the Health Insurance Portability and Accountability Act (HIPAA), and applicable Florida law. Where this Privacy Policy and the Notice of Privacy Practices both apply, the Notice of Privacy Practices controls for PHI.

1. Who We Are

Lifeline Health is a nonprofit public health organization providing no-cost testing, treatment, prevention, and patient support for Hepatitis C, HIV, and other sexually transmitted infections.

2. Information We Collect

Information you provide to us. When you contact us, request an appointment, ask a question, sign up for updates, or complete a form on the Websites, you may provide information such as your name, contact details, date of birth, and the nature of your inquiry. Some forms may ask for health-related information so we can route your request appropriately. Information you submit through our secure intake and appointment forms is treated as protected health information and handled under our Notice of Privacy Practices, HIPAA, and Florida law.

Information collected automatically. When you visit the Websites, we and our service providers may automatically collect technical information such as your IP address, browser type, device type, pages viewed, referring website, and the dates and times of your visits. We collect this through cookies and similar technologies.

We do not knowingly collect more health information through general web forms than is necessary to respond to your request. Please do not send detailed health information through unsecured email or social media. Use only the secure forms we provide.

3. Cookies and Tracking Technologies

The Websites use cookies and similar technologies to operate the site, remember preferences, and understand how the Websites are used. You can set your browser to refuse cookies or alert you when cookies are being sent, though some parts of the Websites may not function properly as a result. We do not use cookies to identify you by name or to track your health conditions.

We use the information we collect to respond to your inquiries and appointment requests, to provide and improve our services and the Websites, to communicate with you by telephone, text message (SMS), email, or mail regarding programs, services, appointments, treatment, and resources, to maintain the security of the Websites, and to comply with legal obligations.

6. How We Share Information

We do not sell your personal information. We share information only as follows:

Service providers and business associates. We may share information with vendors who perform services on our behalf, such as website hosting, secure form processing, and analytics. Where these vendors handle PHI, they are bound by HIPAA business associate agreements.

Legal and safety requirements. We may disclose information when required by law, in response to a valid legal process, or to protect the rights, safety, or property of Lifeline Health, our patients, or others. Disclosure of HIV, STI, and other health information is further restricted by HIPAA and Florida law as described in our Notice of Privacy Practices.

With your direction. We may share information when you ask us to or authorize us to do so.

7. Health Information and HIPAA

Health information you provide as a patient or prospective patient is protected health information. Our collection, use, and disclosure of PHI is governed by HIPAA and by Florida law that provides heightened confidentiality for HIV and sexually transmissible disease information. Please review our Notice of Privacy Practices for a full description of your rights and how we handle PHI.

8. Data Security

We maintain administrative, technical, and physical safeguards designed to protect the information we collect. Our secure intake forms are designed to handle health information in accordance with HIPAA. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security. You are responsible for protecting any login credentials we provide to you.

9. Data Retention

We retain information for as long as necessary to fulfill the purposes described in this Privacy Policy, to provide services, and to comply with our legal, regulatory, and recordkeeping obligations. Medical records are retained according to applicable Florida and federal record retention requirements.

10. Your Choices

You may decline to provide information, though this may limit our ability to respond to you. You can manage cookies through your browser settings. You may request access to or correction of personal information by contacting us. Rights specific to your protected health information, including the right to access and amend your medical record, are described in our Notice of Privacy Practices.

11. Children’s Privacy

The Websites are intended for a general audience and are not directed to children under 13. We do not knowingly collect personal information from children under 13 through the Websites. If you believe a child under 13 has provided us information through the Websites, contact us so we can remove it.

12. Third-Party Links

The Websites may link to third-party websites and resources. We are not responsible for the privacy practices or content of those sites. Review their privacy policies before providing information.

We may update this Privacy Policy from time to time. The “Last Updated” date reflects the most recent version. Material changes will be posted on the Websites.
